Hello Family Council contacts!
The Ontario Health Quality Council (OHQC) has recently launched the Long-Term Care reporting site. Read the message from OHQC below.
On Friday, January 22nd, the Ontario Health Quality Council (OHQC) launched the Long-Term Care Reporting site at www.ohqc.ca
This site provides information on key areas where long-term care homes are measuring quality. It features:
- Provincial averages for 33 indicators of quality in long-term care, such as the percentage of residents with recent unintended weight loss.
- Individual results for over 70 homes related to falls, incontinence and pressure ulcers.
The site is a prototype and is in its first phase of development because information is not available on all homes yet. At this time, its main purpose is to encourage residents, families and staff in homes to discuss this information together and work cooperatively on ideas to support quality improvement. To that end, the site includes some suggested questions for residents and families (see Questions for Residents and Families to Ask).
The goal is to develop the site into a resource that people who are considering long-term care for themselves or a loved one can use as one of many resources available to them when choosing a home. It is anticipated that all homes will be reporting their results at an individual home level on a broad range of indicators by the end of 2011.

The Alzheimer Society of Canada has released a report titled 'Rising Tide: the Impact of Dementia on Canadian Society.' This report explores the health and economic impacts of dementia over the next 30 years, the possible intervention scenarios and their effects, reviews policy options and makes recommendations.

This is a reminder that the deadline for the Family Councils' Program Winter 2010 newsletter is quickly approaching. The deadline for submissions is November 16th 2009. The goal is to make it your newsletter, and we invite all Family Council members to submit articles.
The theme for this edition of the Family Councils’ Program newsletter is “humour and fun.” We are looking for articles (200-350 words) that focus on ways that your Family Council and Long-Term Care Home promote, honour or focus on humour and fun. Examples include fun activities or programs, initiatives that include humour, or everyday examples of fun and humour. Does your Council have a story to share? Contact the Family Councils’ Program Resource Coordinator for details and to submit your article!
We also welcome short articles (100-250 words) about your Council’s recent activities or projects, educational pieces, jokes or inspirational quotes.

Examples include local schools, churches or religious groups, a department or volunteers in your Home, an educational or music group and so on. Does your Council have a creative partnership to share? New sections have been added to the Family Councils' Program website!
More and more Family Councils are creating their own websites and now Councils can request to have their website link available on the Family Councils' Program website. To view the link that is already available, choose "Local Council Documents" from the menu on the left, then select "Family Council Websites and links". If your Family Council has created a website and would like to have the link posted on the Family Councils website, please contact the Resource Coordinator, Samantha Peck, by email (fcpresource@selfhelp.on.ca) or phone (416-487-4355, toll-free 1-888-283-8806) for details!
As well, the Family Councils' Program Introductory film is now available on this website. The Family Councils' Program Introductory DVD provides information about what a Family Council is, stories about family members and staff, outlines the positive benefits of having a Family Council in your Home, and much more! It is a valuable resource for any Family Council!

Self Help Resource Centre (a partner with Concerned Friends in delivering the Family Councils' Program) is pleased to announce that Kelly Potvin will be the new Executive Director at Self Help Resource Centre effective, April 1, 2009.
Kelly has demonstrated expertise in program development and operations, strategic and operational planning, mediation and conflict resolution and management of finance/budgets. She comes with a strong background in community development and has used those skills to build and lead teams, and to promote and foster partnerships which enhance programs and drive organizations. Her successes also include grant proposal funding and the management of change in staff teams and organizations.
Kelly has spent most of her career working in the Community Mental Health sector, with the last eight years in a management capacity. Kelly was recently employed at Reconnect Health Services. Prior to that, she worked at Houselink Community Homes, Sistering Drop-In for Women and Huronia Transition Homes. Most recently, Kelly was the Interim Director for the Ontario Early Years Centre at the Massey Centre for Women.
We look forward to Kelly’s strategic leadership and management working with the staff, board and partners of Self Help Resource Centre.

Updated information about the RAI-MDS program is now available!
Community Care Information Management (previously known as the Continuing Care e-health) is the program that delivers Common Assessments and Business Systems projects across the long-term care, community mental health, CCACs and Community Support Services sectors by leveraging the leadership role of LHINs. Long-Term Care Homes Common Assessment Project (LTCH CAP) is one of the projects under this program’s umbrella and facilitates and supports the RAI-MDS program. The Connections newsletters features an article on LTCH CAP.

IMPORTANT MESSAGE FROM ELECTIONS CANADA ABOUT THE NEW IDENTIFICATION RULES FOR VOTERS
A federal election will be held on Tuesday, October 14th. Last year, the Canada Elections Act was changed. To vote, electors must now prove their identity and address by providing:
One original piece of identification issued by a government or government agency containing their photo, name and address, such as a driver’s licence; or
Two original pieces of identification, authorized by the Chief Electoral Officer of Canada (listed on Elections Canada website). Both must show their name and one must include their address, such as a health card.
For electors living in Long-Term Care Homes, it may be a challenge to provide the necessary identification. They may not have it in their possession, or they may have a health card or hospital card, but not any identification that shows their current address.
Residents who do not have identification that proves their current address can use an Attestation of Residence (issued by the Long-Term Care Home). This form can be used to prove their address when they vote. Another piece of identification showing the resident’s name will also be required. If your loved one does not have any identification showing their current address, please inquire with the Long-Term Care Home about the Attestation of Residence Form.
Another option for residents is to be vouched for at the polling station by another elector who is already registered on the list of electors in the same polling division and who provides acceptable piece or pieces of identification. Both will be required to make a sworn statement. An elector cannot vouch for more than one person, and the person who has been vouched for cannot vouch for another elector.

Nominations of Family Council members from across the province are welcome for three current openings on the Family Councils’ Program Advisory Committee. The mandate of the FCP Advisory Committee is to support and promote the goals and objectives of the Family Councils' Program.
The Committee's Terms of Reference state that there may be up to five Family Council members on the Committee. Other Committee members include representatives from Concerned Friends, The Self-Help Resource Centre, the Ontario Association of Residents' Councils, The Ontario Association of Non-Profit Homes and Services for Seniors and The Ontario Long-Term Care Association. A representative from the Program's funding authority is an ex-officio committee member.
Advisory Committee members meet with FCP staff to share information, engage in discussion, provide advice and make recommendations to the Program.
Meetings are held in Toronto with Committee members outside Toronto participating via teleconference. Under the Terms of Reference, nominations for new members must be presented to the Advisory Committee for consideration.

The Results from the Family Council survey have been compiled into a document for planning and discussion purposes. The end result of this process is to have a plan with strategic directions that can guide the Regional Networks and the Program over the next three years. The document is rich in detail and the content speaks strongly of many issues that many Regions and Family Council members have raised as issues at regional meetings and to the program staff.
Planning is a process and there are many steps before we have a plan that has consensus and can be implemented. We hope that this document can be a tool to build the consensus and align all the work that family councils across the province are involved in .
The next steps in the process:
· We are posting this plan here on the website so that the regions and all who participated in the survey can begin to think about this document, discuss the material and how it pertains to their region.
· The regional chairs will meet together in the fall and discuss in detail the strategic directions brought forward in the document and begin to map out how these show up ( or not) in their regions and how it can help direct the work over the next year.
We encourage you to read this, discuss it and talk to your regional networks about it. Feel free to send us further comments and suggestions. Planning is a continuing process and we are in the beginning phase of the process and look forward to working with everyone on this important work.

The Ministry of Health and Long-Term Care announced on August 19 2008 that they will be providing $23.3 million to support the creation of 873 personal support worker positions in long-term care homes. This will increase the average paid hours of direct daily care per resident to 3.26 hours this year of nursing, personal support and programming.

The Family Council Member survey is now closed. The Family Councils' Program thanks everyone who participated. Family Council Member Survey
The Family Councils’ Program is undertaking a survey of family council members to help strengthen the effectiveness of the family council networks. The results of this survey will be compiled by an independent consultant who will identify emerging themes and issues. These issues will be used to plan future directions for the next two years. A corresponding implementation plan, that will guide the work of the Program and Networks, is expected to be developed in the fall of this year.
History of Development
The Family Council Project began ten years ago with funding from the Ontario Trillium Foundation to pilot several family councils in selected Long Term Care homes. Once the pilot was finished the Ministry of Health and Long Term care decided to fund the Family Councils Program in 2004. The program mandate is to develop and maintain family councils across the province. The networks were established to link up councils in each region through sharing information and regular regional meetings.
Family Council Program Staff
The program is one of several that is facilitated through the Self Help Resource Centre, with three staff : An Outreach Coordinator, a Network Coordinator, and a Resource Coordinator.
Where Are We Now?
The Family Council Regional Networks are in different phases of development across the province. Some networks are very active with other regions just forming and beginning to establish regular meetings. Participants in the regional networks change as new families come into councils and subsequently attend regional network meetings, and others leave. It is now time to take a few moments to pause and reflect on where we go from here. This survey is a tool for us to hear from individuals in family councils about their perceptions and expectations of the regional networks and the program. We want to thank everyone who takes the time to fill out our survey and submit it. Your opinions and comments matter to us!
Please submit the survey anytime before noon on Monday July 21, 2008 by clicking on the survey link provided below. The survey should take approximately 15 minutes of your time to complete. Please join me in welcoming Lorraine to the Family Councils Program staff team.
Lorraine Purdon is the new network coordinator for the Family Councils’ Program. Lorraine has worked in community Development, planning and capacity building for most of her career. Most recently Lorraine was the Director of The South-east Toronto Planning Project-a coalition of health and social services in the Downtown core of Toronto. Through her work as Director of the coalition she was instrumental in launching the Toronto Oral Health Coalition, was involved in the community consultations during the planning of the Local Health Integration networks by promoting a community voice during the deliberations and worked closely with St. Michael’s Hospital and the Sherbourne Health center in making services more accessible to people who were homeless and marginally housed. Over the past several years Lorraine was part of a Family council in her home region of LHIN 9.
We look forward to working with her! Our team is now complete!

Happy New Year!
The tremendous commitment and efforts put forth by Family Councils during 2007 resulted in great strides with regards to the development of Regional Networks, their recognition by government, and - as always - the local impacts of new and established Family Councils helping to improve quality of life for residents in long-term care.
FCP’s winter newsletter will be off to the printer this week. To make sure that you get your free copy… BY EMAIL: Click on the “E-newsletter” item on the menu to the left (or go to http://www.familycouncilmembers.net/list.html) and type in your email address. BY SURFACE MAIL: Contact the FCP office by phone or email and tell us your name, mailing address, and Family Council “relationship”, e.g. member (family/friend), LTC staff, or other organization.
At the FCP office, we look forward to working with all of you over the coming year!

Through September and October, many Regional Family Council Networks have been busy with Regional Meetings. To find out what's happening in your region, look at the documents under "Regional Networks". They are now easy to access by region. And if you have something your Region is working on that you would like us to post, send it in!

New Discussion Forums! We moved some of the previous dynamic conversations to our new discussion forums. We hope you'll find these new forums easy to use and navigate. Anyone can register, and it's your choice whether to remain anonymous. Please Note: if you were registered on our previous site your old registration won't automatically convert, but do re-register and chat with us!

This new site combines both our old sites so it will be easier to use and find the resources you need. Our discussion and documents areas are still being transferred to the new system, and you can still browse the existing files for references and information.
We hope you enjoy the new site and come back soon to see more changes take effect!
