Welcome to the Family Councils' Program (FCP) Website. FCP offers free information resources, consultation services, networking support and training opportunities - to help you start, strengthen and sustain your Council.

A Family Council is an organized, self-led, self-determining, democratic group composed of family and friends of the residents of long term care homes.

"Families working together to provide support, share experiences, and seek solutions to common problems. This is a Family Council."

The Long Term Care Task Force on Resident Care & Safety has released its Action Plan: An Action Plan to Address Abuse and Neglect in Long-Term Care Homes.

This morning at 11:00 AM the Long Term Care Task Force on Resident Care and Safety released its report at a Press conference held at Belmont House Long Term Care in Toronto. The report has been sent electronically to all Homes in Ontario and to all the member associations of the task force.

On Friday June 15th 2012, the Family Councils’ Program is celebrating Family Council Week with an exciting and interactive event on ”Quality Improvement in Long-Term Care: The Role of Families.” Join us to learn more about quality improvement in Long-Term Care and celebrate the work of Family Councils!

Presented in partnership with Health Quality Ontario, the Family Council’s Program presents an Interactive session with Gayle Stuart, Residents First Program Manager (Health Quality Ontario), on June 15th 2012 1-4pm at The Village of Humber Heights in Etobicoke.

Health Quality Ontario (HQO) has launched their updated and revamped public reporting website on Long-Term Care (LTC) Homes. To view the information on Long-Term Care Homes, click the link below (available in English and French).

As part of HQO's mandate - which also supports continuous quality improvement and promoting healthcare based on the best scientific evidence available - public reporting increases awareness and provide information about the quality of Ontario's long-term care and home care systems. Public reporting also enhances transparency, encourages accountability and drives quality improvement in the provincial healthcare system.

This year's long-term care website features more publicly reporting long-term care homes than ever before - nearly 300 homes have voluntarily posted their results for quality indicators related to instances of incontinence, falls, pressure ulcers and restraints. Users can search for homes by postal code or by local health integration network (LHIN). HQO applauds these homes for being early adopters of public reporting and leaders among their peers.

The long-term care website also includes provincial results for 12 long-term care quality indicators on topics such as wait times, falls, pressure ulcers and emergency department visits. These indicator results are organized around five attributes of a high-performing healthcare system: accessible, effective, safe, appropriately resourced and focused on population health. By the end of 2012, data for all long-term care homes in Ontario will be posted to the site.

Health Quality Ontario (HQO) is a government agency created under the Commitment to the Future of Medicare Act in 2005 and had its mandate expanded in 2010 with the passing of the Excellent Care For All Act (ECFA). In its current role, HQO is dedicated to the establishment of priorities and developing leadership around a unified goal of delivering maximum value to Ontarians from their publicly-funded health system. HQO's accountability is demonstrated through the provision of timely monitoring and measurement of the health system while continuing to assess progress and report this data to the public. The Family Councils' Program is pleased to announce the launch of the new Family Council manual: Your Guide to Starting and Maintaining a Family Council.

The Guide has been revised and updated to reflect the new legislation governing Long-Term Care in Ontario (The Long-Term Care Homes Act, 2007) and other Family Council developments.

If you are accessing the guide on a computer or on-line, hyperlinks have been added to help you locate topics identified in the table of contents. Hyperlinks have also been used to allow you connect on-line to other resources and supports.

Printed copies will be sent to each Long-Term Care Home over the summer and the French translation is currently in progress. If your Home or Family Council would like to receive a French Guide, please contact Samantha Peck, FCP Resource Coordinator, by phone (416-487-4355, toll-free 1-888-283-8806 ext. 25) or email fcpresource@selfhelp.on.ca.
